Payment posting that tells you what was paid, adjusted and taken back.

We post insurance payments claim by claim from ERAs and EOBs, then look for the things that quietly cost a practice money: recoupments nobody explained, reversed payments and claims paid below the fee schedule.

The short answer

What is payment posting, and what is a recoupment?

Payment posting is recording each insurance payment against the claim it pays, in the practice's billing system. The information comes from the ERA (electronic remittance advice) or the EOB (explanation of benefits) the payer sends. Accurate posting is what makes your patient balances, your AR and your revenue reports correct.

A recoupment is money the insurer takes back from a payment it made earlier, usually by deducting it from a later payment. A reversal is a payment the insurer cancels, often to reprocess the claim. Both are easy to miss on a busy remittance, and an unexplained recoupment can hide an error by the payer.

Our team posts payments in NextGen and SimplePractice from ERAs and EOBs downloaded from payer portals or Availity. We enter each claim's paid amount, patient responsibility and contractual adjustments, and review the ERA and claim history for adjustments, reversals and recoupments. When a recoupment is unclear, we ask the payer and handle it based on the reason they give. We also compare the allowed amount with your fee schedule to catch underpayments, and file an appeal or reconsideration with supporting documents when one is justified.

Questions

What practices and plans ask us.

Which billing systems do you post payments in?
NextGen and SimplePractice. We download ERAs and EOBs from payer portals or Availity.
What is the difference between a recoupment and a reversal?
A recoupment is money the insurer takes back from an earlier payment, usually by deducting it from a later one. A reversal cancels a payment, often so the claim can be reprocessed.
What do you do with a recoupment you do not understand?
We review the ERA and the claim history, then ask the payer for the reason and handle it based on what they say.
Do you check for underpayments?
Yes. We compare the allowed amount with the expected payment under your fee schedule, and file an appeal or reconsideration with supporting documents when the payment is short.
How long does posting take?
2 to 5 minutes per claim. Overall turnaround depends on your volume and on when the ERA or EOB is available.
