Denial management and AR follow-up for claims that should have been paid.

Unworked denials and aging claims are money your practice has already earned. We sort denials by reason, call the insurer when the portal does not explain enough, file appeals for incorrect denials, and work your oldest unpaid claims first.

The short answer

What is denial management?

Denial management is the work of finding out why an insurer refused to pay a claim, fixing the cause, and getting the claim paid through a corrected claim, a reconsideration or an appeal. AR follow-up is the related work of chasing claims that have not been paid or answered, so they do not age past the payer's deadlines. AR stands for accounts receivable: money owed to the practice.

We start by sorting denials by reason, such as a missed timely filing limit, a missing authorization or an absent referral, because each reason has its own fix and each payer has its own rules. We review the denial in the insurance portal, and call the insurer when we need more detail. On the call we explain the issue, ask how it can be resolved, and request a review when one is appropriate.

For denials we believe are wrong, we file an appeal through the payer portal with supporting documents. For AR follow-up we use your AR ledger to find outstanding claims, check their status and age, and work the oldest first. Every action and every call reference number is logged in your system, so you can see what was done on any claim.

How we work

How we work a denial.

Each claim ends with a clear outcome: corrected and resubmitted, appealed, sent for payer review, or a named next action.

  1. 1
Sort by reasonTimely filing, missing authorization, missing referral and other reasons, each handled by the payer's rules.
  2. 2
Review in the portalWe read the denial details in the insurance portal before anything else.
  3. 3
Call the insurerWhen the portal is not enough, we call, explain the issue, ask about the resolution and request a review where appropriate.
  4. 4
Correct or appealCorrected claims are resubmitted. Incorrect denials are appealed through the portal with supporting documents.

Questions

What practices and plans ask us.

How do you decide which claims to work first?
We use your AR ledger to find outstanding claims, check their status and age, and work the oldest first, since those are closest to the payer's filing and appeal deadlines.
Do you call insurance companies?
Yes. When the portal does not explain a denial, we call the insurer, explain the issue, ask how to resolve it and request a review where appropriate. Every call reference number is logged.
Do you file appeals?
Yes. For denials we believe are incorrect, we submit an appeal through the payer portal with supporting documents and track it.
How long does it take to work a denial?
The first action on a simple denial takes 15 to 30 minutes. A complex denial takes 30 to 45 minutes.
Can we see what was done on each claim?
Yes. The claim details, the action taken and the call reference number are logged in your system.
